Comparison of Air and Bone Conduction Thresholds in Normal and Conductive Hearing Loss using ASSR, ABR, PTA

Abstract
Auditory brainstem response (ABR) is frequently chosen to objectively estimate hearing thresholds at audiometric frequencies for the individuals who cannot be tested using conventional behavioral measures (PTA). This objective technique, however, requires skilled judgement of correct responses especially in young infants and results generally lack specificities in low frequency regions (<1 kHz). Auditory steady state response (ASSR) is currently of great interest since responses seem to indicate some frequency specific thresholds in both air and bone conduction stimulations and correct responses are determined with statistical decision rules. The purpose of this study was to investigate air-bone gaps in normal hearing subjects and in conductive hearing subjects using ASSR, ABR, and PTA with special reference to ASSR as a prospective measure of the air-bone gap. Data were obtained from 10 ears with normal hearing and 9 ears with conductive hearing loss. Results showed that air and bone conduction thresholds in ABR and ASSR were elevated significantly more than thresholds in PTA at 500 Hz, 1 kHz, 2 kHz, and 4 kHz. Howerevr, air-bone gaps in PTA, ABR, and ASSR were not statistically different in all three methods. Differences in absolute thresholds between ASSR and PTA suggested the inclusion of appropriate correction factors. Results also indicated that ASSR could be a useful objective tool for conductive hearing loss in the cases where conventional behavioral measures were difficult to achieve.
